Registered Nurse in Home Health providing and directing nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinates total plan of care with other health care professionals. Responsibilities include developing and implementing plan of care, supervising home health aides and LPNs, completing comprehensive assessments (OASIS), documenting patient visits, communicating with physicians and families, participating in on-call and weekend rotation, and reducing unnecessary patient hospitalizations.
Requires current unrestricted RN licensure in state of practice, current driver's license and vehicle insurance, and 1+ years clinical experience as an RN in Louisiana.
