Responsible for Diabetes Mellitus (DM) instruction, documentation, and follow-up on patients identified through physician referral. Conducts quality and effective diabetes self-management education program classes. Accountable for ongoing evaluation of program, performance improvements, physician and patient phone calls/correspondence. Primarily responsible for outpatient appointments. Develops, implements, and evaluates community educational events. Participates in community diabetes screenings. Maintains ADA or CDCES Diabetes Recognition Program status.

Requirements

Bachelor's Degree in Nursing required; Master's Degree in a related field preferred; 1 year of experience in the related field preferred; RN licensure in the state of employment required; BLS required; Certified Diabetes Care and Education Specialist (CDCES) or Board Certified - Advanced Diabetes Management (BC-ADM) required within 24 months of hire; Certified Pump Trainer (CPT) preferred
