Wound Care Nurse (RN/LPN)

Boca Circle Rehabilitation Center is seeking a dedicated and compassionate Wound Care Nurse (RN/LPN) to join our clinical team. In this vital role, you will collaborate with an interdisciplinary team (IDT) to deliver high-quality wound and skin care services, ensuring optimal comfort, healing, and quality of life for our residents. Key responsibilities include collaborating with the IDT to plan, implement, and evaluate individualized resident care plans, assisting with completion of applicable sections of the MDS, reviewing and following skin and wound care policies, assessing and treating wounds, managing risk factors for pressure ulcers, performing routine skin assessments, educating residents and families, evaluating nutritional needs, recommending pressure-relieving devices and wound dressings, and reporting any suspicion of abuse or neglect.
