Job Title: LPN Location: Tulsa, OK
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Tulsa Center for Rehabilitation & Healthcare, a skilled nursing and rehabilitation community serving Tulsa and the surrounding area. As an LPN at our facility, you will provide direct nursing care to our residents, supervising the day-to-day nursing activities performed by certified nursing assistants, and ensuring that our residents receive high-quality care that meets their individual needs.

Responsibilities
Provides direct nursing care to residents, including administering medications, performing wound treatments, and providing assistance with activities of daily living (ADLs)
Supervises the day-to-day nursing activities performed by certified nursing assistants (CNAs) and medication technicians/aides
Completes accurate and timely documentation of resident care and services
Participates in the admission, discharge, and transfer process of residents
Observes and reports changes in residents' status to the physician and resident's family or representative
Collaborates with other members of the interdisciplinary team to ensure comprehensive care that meets the needs of our residents
Participates in quality improvement initiatives to enhance the quality of care and services provided
Maintains a safe and clean environment for residents, staff, and visitors
Required
Qualifications
A Nursing Degree from an accredited college or university or a graduate of an approved LPN/LVN program
Prior experience in long-term care preferred but not required
Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in practicing state
Current CPR certification
Major Duties and
Responsibilities
Directs the daily activities of CNAs in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures
Ensures that policies and procedures are complied with by nursing personnel assigned
Participates in the survey process when inspection surveys arise
Maintains confidentiality of protected health information, including verbal, written, and electronic communications
Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel
Additional Tasks
Treats all residents with dignity and respect
Promotes and protects all residents' rights
Establishes a culture of compliance by adhering to all facility policies and procedures
Complies with standards of business conduct and state/federal regulations and guidelines
Personal Skills and Traits Desired
Ability to read, write, speak, and understand the English language
Ability to make independent decisions when circumstances warrant such action
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
Must possess leadership and supervisory ability and willingness to work harmoniously with and supervise other personnel
Must be able to follow oral and written instructions
Ability to communicate effectively with medical and nursing staff, and other departments
Compliance as a Condition of Employment and Performance Appraisal
Agreement to abide by all standards, policies, and procedures of the facility, including the facility's compliance and ethics program, is a condition of employment
Compliance will be a factor in evaluating job performance
Violations, including failure to report violations, will result in disciplinary action, up to and including termination
