We are seeking a dedicated *Licensed Practical Nurse (LPN)* to join our team full-time and support residents within partner facilities.

Overview
* The LPN will play a vital role in bridging patient care with technology by monitoring vital signs, facilitating telehealth visits, and being a point of communication with residents enrolled in our RPM and CCM programs.

Responsibilities
*
- Take and record vital signs for residents enrolled in the RPM program using company-provided equipment.
- Facilitate telehealth visits and calls between residents and Nurse Practitioners (NPs).
- Conduct RPM and CCM tasks for assigned patients, either remotely or in a quiet designated area of the facility.
- Maintain accurate documentation of all patient interactions and vitals.
- Collaborate with NPs, facility staff, and our health care team to ensure seamless quality care.
