We provide skilled home health services to Medicare and privately insured patients with a focus on quality care, responsive clinical support, and continuity of care.

Position Summary

The Home Health Registered Nurse provides skilled nursing services in patients' homes, including comprehensive assessments, development and implementation of individualized Plans of Care, skilled nursing interventions, patient and caregiver education, care coordination, and timely clinical documentation.

RNs assigned OASIS visits will complete required OASIS assessments electronically using WellSky/Kinnser.

Key Responsibilities

  • Perform comprehensive nursing assessments in the patient's home.
  • Complete OASIS assessments, including Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge assessments, as assigned.
  • Develop and update individualized Plans of Care based on patient assessments, physician orders, and identified clinical needs.
  • Provide skilled nursing interventions, treatments, medication management, and patient/caregiver education.
  • Monitor changes in patient condition and communicate significant findings to physicians and the interdisciplinary care team.
  • Coordinate care with physicians, therapists, LVNs, HHAs, caregivers, facilities, and other healthcare providers.
  • Serve as a patient advocate and promote continuity and coordination of care.
  • Supervise LVNs and Home Health Aides as required by the Plan of Care and agency policy.
  • Complete clinical documentation accurately and within agency-required timeframes.
  • Participate in case conferences, staff meetings, orientation, and required in-service education.
  • Follow agency policies and applicable Medicare, state, and federal requirements.

Qualifications

Required

  • Current and unrestricted Texas Registered Nurse (RN) license
  • Minimum 1 year of recent adult home health nursing experience
  • Previous OASIS experience
  • Ability to independently complete OASIS assessments and associated clinical documentation
  • Current CPR certification
  • Valid driver's license and reliable transportation
  • Ability to travel to patient homes throughout Collin County
  • Strong clinical assessment, communication, documentation, and time-management skills
  • Ability to work independently in a field-based environment

Preferred

  • Experience working for a Medicare-certified home health agency
  • Experience with WellSky/Kinnser

Compensation

This is a PRN, pay-per-visit position.

Current visit rates include

  • OASIS Start of Care (SOC): $150 per visit
  • Routine Skilled Nursing Visit: $50 per visit
  • Additional visit types and rates will be discussed during the interview process.
  • Orientation and training are compensated hourly.
