What You'll Do
As a Home Care Registered Nurse (RN), you'll provide skilled nursing care in patients' homes, including:
Conducting comprehensive nursing assessments and developing individualized plans of care Completing OASIS Start of Care, Resumption of Care, Recertification, and Discharge assessments Wound care and dressing changes Medication management and patient education Catheter, ostomy, and other skilled nursing care Disease management and chronic care education Coordinating care with physicians and interdisciplinary healthcare teams Monitoring patient progress and identifying changes in condition Ordering medical supplies as needed Completing accurate, timely electronic documentation
