We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will collaborate with a multidisciplinary team to develop and implement individualized care plans while promoting patient independence and quality of life. Duties include conducting comprehensive assessments, administering medications and treatments including IV insertion and catheter care, utilizing EHR systems, educating patients and families, collaborating with physicians and healthcare professionals, monitoring patients for changes in condition, and staying updated on ICD-9 and ICD-10 coding.

Requirements

  • Current Registered Nurse (RN) license in the state of practice
  • Previous experience in home health care or a teaching hospital setting is preferred
  • Familiarity with electronic health record systems such as eClinicalWorks or Epic is a plus
  • Strong clinical skills, particularly in IV insertion and catheter care
  • Excellent communication skills for effective patient education and collaboration with healthcare teams
  • Ability to work independently while managing multiple patient cases efficiently
