About this role
Optimal Home Care is looking for a Registered Nurse to provide services for our patients. Responsibilities include completing SOC/ROC/RECERTs, supervisory visits, managing care coordination, supervising LPNs, fiscal management of patient cases, and collaborating with marketers and facility staff. anticipated work hours minimum 40 hours per week, typically Mon-Fri.

Requirements
- Graduate of an accredited, approved school of professional nursing
- Active license in the State of Colorado in good standing
- Must be able to communicate in the language of the client and document client related activity as appropriate
- Current driver's license
- At least one (1) year of experience as a Registered Nurse
- Home Health experience is a plus
- Familiarity with OASIS preferred
