*Duties*
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Point Click Care.
- Administer medications and injections accurately following physician orders, including IV infusions and catheter care.
- Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
- Manage acute pain and chronic conditions through appropriate interventions, including wound care, tube feeding, and ventilator management.
- Provide specialized care for patients with dementia or Alzheimer’s disease in memory or geriatric care settings while ensuring safety and dignity.
- Support discharge planning and coordinate with case management teams to facilitate smooth transitions of care across settings.
- Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting medical records accurately in PCC.
*Skills*
- Proven experience working with diverse elderly patient populations.
- Strong knowledge of medical terminology, anatomy, physiology.
- Proficiency in using PCC.
- Knowledge of infection control practices and experience managing infectious disease cases; familiarity with ICD-10 coding enhances documentation accuracy.
- Excellent caregiver communication skills to educate patients and families about treatment plans and health coaching strategies.
*Qualifications*
- Proven experience within healthcare and rehab units.
- Excellent organizational skills with the ability to oversee multiple patients
- Valid nursing license and relevant certifications demonstrating professional competency
