*Shifts*

• 3pm-11pm
• 11pm-7am

*Duties*

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Point Click Care.
• Administer medications and injections accurately following physician orders, including IV infusions and catheter care.
• Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
• Manage acute pain and chronic conditions through appropriate interventions, including wound care, tube feeding, and ventilator management.
• Provide specialized care for patients with dementia or Alzheimer’s disease in memory or geriatric care settings while ensuring safety and dignity.
• Support discharge planning and coordinate with case management teams to facilitate smooth transitions of care across settings.
• Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting medical records accurately in PCC.

*Skills*

• Proven experience working with diverse elderly patient populations.
• Strong knowledge of medical terminology, anatomy, physiology.
• Proficiency in using PCC.
• Knowledge of infection control practices and experience managing infectious disease cases; familiarity with ICD-10 coding enhances documentation accuracy.
• Excellent caregiver communication skills to educate patients and families about treatment plans and health coaching strategies.

*Qualifications*

• Proven experience within healthcare and rehab units.
• Excellent organizational skills with the ability to oversee multiple patients
• Valid nursing license and relevant certifications demonstrating professional competency
