As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. Responsibilities include providing skilled nursing care to patients in their homes (administering medications, performing wound care, conducting patient assessments), developing and implementing individualized care plans, monitoring patients' conditions, educating patients and families on disease management, maintaining accurate records, and overseeing Home Health Aides.

Requirements

  • Med surg, ICU, ER, or acute experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • BLS Certification
  • Experience collaborating with a team of healthcare professionals
  • Valid driver's license, auto insurance and reliable transportation
  • 1 year nursing experience (preferred)
  • Home Health experience (preferred)
