CaringHand Home Health Solutions is seeking compassionate and dependable Physical Therapists (PTs) and Registered Nurses (RNs) to join our growing home health team in Nevada County, CA. Responsibilities include providing physical therapy services, conducting patient assessments and skilled nursing visits in patients' homes, developing and updating individualized plans of care, administering medications and treatments, monitoring patient conditions, educating patients and caregivers, coordinating care with physicians and interdisciplinary teams, and maintaining accurate clinical documentation.

Requirements

Active California Registered Nurse (RN) license
Active California Physical Therapist license
Active California Registered Nurse (RN) license
Active California Physical Therapist license
Graduate of an accredited nursing program
Degree from an accredited Physical Therapy program
Minimum 1 year RN experience preferred
Minimum 2 years of PT experience preferred
Home health or hospice experience is a plus
Current CPR certification
