About this role
Transitions LifeCare is looking for an RN Case Manager to join our team. This is a full time position Monday - Friday, 8am-5pm with occasional call.
We are looking for a competent, caring, professional RN with strong assessment and organizational skills to join our team. We service a 50 mile radius of the greater Tulsa area and we see patients where ever they call home. As a part of our patient team, RNs, LPNs, Social Worker, Chaplain, Hospice Aides, and Volunteers work together as we provide compassionate care.
