About this role

All-4-One HHS, Inc. is looking for Full-Time and Part-Time Per Diem availability for RN's to join our growing CHAP Accredited agency to provide home visits for our patients and facilitate their speedy recovery. The ideal candidate will be a responsible professional able to give the best nursing care with little supervision to do admissions, recerts and resumption of care and routine visits which include medication evaluation and monitoring, wound care, diabetic teaching, etc. in compliance with the patients 485/POC. The goal is to promote patient’s well-being by providing high quality nursing care.

RN will be familiar with MCR regulations. RN will be responsible for initial and on-going comprehensive assessment for OASIS admission for client services. Develop the Plan of Care for services needed and providing those services and/or treatments required. Responsible to coordinate services and supervising of LPN/LVN's and paraprofessionals.

  • Monitor patient’s condition and assess their needs to provide the best possible care and advice
  • Observe and interpret patient’s symptoms and communicate them to the MD
  • Inspect and maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.

*Skills*

  • Proven experience as a registered nurse
  • Excellent knowledge of nursing care methods and procedures
  • In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
  • A team player with excellent communication and interpersonal skills
  • Responsible
  • Outstanding organizational and multi-tasking skills
  • Excellent problem-solving skills
  • BSc or other diploma from a nursing program
  • Valid nursing license
