The Hospice Liaison Nurse serves as a vital connection between patients, families, healthcare providers, and hospice services. In this role, you will conduct clinical assessments, facilitate hospice admissions, and coordinate discharge planning from acute inpatient settings. You will collaborate closely with physicians, case managers, healthcare partners, patients, and their loved ones to ensure seamless transitions to hospice care and support patient-centered outcomes. This position is primarily hospital-based, with occasional field admissions as needed.

Requirements

High school diploma or GED (required)
Graduate of an accredited school of nursing (required)
Bachelor of Science in Nursing / BSN (preferred)
Current RN licensure in appropriate state (required)
Minimum 3 years experience in home care or hospice (required)
Home care experience
Hospice experience
Acute care experience
