REGISTERED NURSE - RN - CASE MANAGER - HOSPICE - Saginaw, MI and Surrounding areas: Amara Hospice Care is looking to hire a Registered Nurse (RN) Case Manager to join our team of professionals for Saginaw and the surrounding areas. The RN Case Manager will be responsible for assisting the Team with Case Management/Visits to aid in coverage and call outs. Job duties include completing initial assessments, re-evaluating patient nursing needs, collaborating with the healthcare team, counseling patients and families on end-of-life needs, supervising Hospice Aides and LPNs, and participating in on-call rotation.
Requirements
Current Michigan RN License in good standing
Hospice experience
