Registered Nurse (RN) Case Manager – Floating Territories role at Home Health Care, Inc. Float between home health territories throughout the Twin Cities based on patient needs. Complete skilled nursing visits, OASIS assessments, and individualized care plans. Coordinate care with physicians and interdisciplinary team members. Full-Time, Monday–Friday, 8:00 AM – 5:00 PM with on-call rotation. Greater Twin Cities Metro Area (21-county service area).
Requirements
- Active Minnesota Registered Nurse (RN) license
- Strong clinical judgment and adaptability
- Comfortable working independently in a field-based environment
- Home health experience preferred, but not required
- Reliable transportation and willingness to travel throughout the metro
