About this role
Registered Nurse (RN) - Coordinate, implement, and supervise all nursing and personal care services at CBAS/ADHC. Provide skilled nursing care. Conducts nursing assessments and develops a Person-Centered Plan of Care. Monitors vital signs, conducts care coordination, provides health counseling, develops training programs, refers participants to community health care agencies, coordinates personal care activities, completes quarterly progress notes, collaborates with Staff Physician, supervises OT/PT Maintenance Program, conducts chart audits, oversees nutrition and kitchen services, supervises medication activities, and maintains privacy rights of participants. Minimum qualifications include knowledge of public health nursing principles, physical assessment techniques, care planning methods, health counseling techniques. Satisfactory completion of an approved nursing school with current license to practice, preferably as a Public Health Nurse. Experience working with frail and chronically ill elderly persons in a multidisciplinary team.
