About Us
Seasons Home Health Care is a clinician-owned home health agency dedicated to delivering exceptional, 1-on-1 care in the comfort of our patients’ homes. Our mission is to provide the highest quality of clinical care and customer service in a patient-focused environment. The area of coverage is Dowagiac, Niles, and Berrien Springs.
What You’ll Do
- Provide 1-on-1 skilled nursing care directly in patient homes.
- Collaborate with physicians regarding medication management and updates to the plan of care.
- Manage and coordinate multi-disciplinary care teams (LPNs, Home Health Aides, physical/occupational therapists).
- Accurately complete OASIS documentation, recertifications, resumptions of care (ROCs), and discharges within an EMR system.
- Proactively assess patient changes to prevent unnecessary re-hospitalizations.
- Educate and counsel patients and family members on disease processes, injury management, and self-care routines.
- Attend required orientation, agency meetings, and continuing education (CEU) opportunities.
What We’re Looking For
Licensure
Active Registered Nurse (RN) license in the state of Michigan.
Certifications
Current CPR certification.
- Transportation: Valid driver’s license and current auto insurance.
Experience
Previous home health, hospice, or related clinical nursing experience preferred.
Skills
Proficiency with EMR documentation, strong organizational skills, and excellent interpersonal communication.
