About this role

My Home Hospice is seeking an experienced, compassionate, and motivated Hospice Clinical Liaison RN / Admissions Nurse to join our growing team. This hybrid position combines business development, community outreach, clinical assessment, and hospice admissions to help patients and families access compassionate end-of-life care. The ideal candidate is passionate about building trusted relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and other healthcare providers while utilizing strong clinical expertise to assess hospice eligibility, educate patients and families, and coordinate seamless admissions into hospice services.

This role is ideal for an RN who enjoys both community engagement and direct patient care while making a meaningful difference in the lives of others.
