Registered Nurse position serving as a Patient Aligned Care Team (PACT) Care Manager at the VA, providing comprehensive, coordinated care for primary care patients in an interdisciplinary, team-based environment. Duties include managing care coordination for a designated panel of patients, emphasizing health promotion, chronic care management, patient self-management support, and overall wellness.

Requirements

  • Current, full, active, and unrestricted RN license in a State, Territory or Commonwealth of the United States, or the District of Columbia
  • Graduate of a school of professional nursing accredited by ACEN or CCNE
  • English language proficiency required
  • U.S. Citizenship required
  • Background/security investigation required
  • Drug screening required
  • Pre-employment physical evaluation required
  • Seasonal influenza vaccination required
