About this role
Looking for an ACHC Accredited & 2016 Home Care Elite top 25% Home Health Agency to work for. Part-time or PRN, RN Field Nurse position immediately available for Cedar Park and surrounding areas. As a Home Health RN, you will provide admission, case management, Resumption of Care visits and follow-up skilled nursing visits.
Interact and collaborate with physicians to develop the initial plan of treatment. Coordinate appropriate care with the multidisciplinary team including Physical Therapy, Occupational Therapy, Respiratory Therapy and Home Health Aides. Report patient care, condition, and progress to physician and DON.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and LVNs. Review medical documentation on all patients in your case load.
