Full-time and part-time Hospice RN Case Managers Company: Hospice Services of Massachusetts
We are seeking an experienced and compassionate RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.
Up to $51 an hour, depending on experience, with weekly pay.
This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel to portions of Barnstable and Plymouth Counties.

Responsibilities
Supervise hospice aides
Complete comprehensive assessments of patient and family to determine hospice needs
Provide professional nursing care by utilizing all elements of the nursing process
Develop and implement individualized care plans
Collaborate with interdisciplinary team members to ensure comprehensive care
Provide health care instructions to patients and families
Counsel patients and families on meeting nursing and related needs
Requirements
Graduate of an accredited school of nursing
Must possess an active and unencumbered RN license in Massachusetts
1-2 years of recent acute care experience in an institutional setting, and 1-2 years of recent experience in home care preferred
CPR certified
