We are seeking an experienced Full-Time Hospice Registered Nurse (RN) Case Manager to join our team and serve patients throughout the Topeka, Kansas area. As a Hospice RN Case Manager, you will provide skilled nursing care to hospice patients in their homes, develop individualized care plans, and collaborate with an interdisciplinary team to ensure the highest quality of care. A minimum of one (1) year of hospice experience is required for this position.
