Schedule:
PRN as needed. Responsibilities include conducting patient and family assessments, administering medications and treatments per physician orders, recording clinical notes, coordinating with community health partners, and maintaining clinical skills through continuing education.

Requirements
Active RN license in the state of employment
Valid driver's license
Active RN license in the state of employment
Graduate of an accredited school of Nursing
Clinical experience in hospice, home health, or a related setting preferred
Flexibility and confidence working across varied patient assignments
Strong assessment skills and sound clinical judgment under pressure
Excellent documentation habits and communication skills
Valid driver's license and willingness to travel locally
