We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home health care team. - Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications, injections, IV infusions, and other treatments following prescribed protocols while maintaining strict adherence to HIPAA and infection control standards.
- Manage complex medical procedures such as catheterization, tube feeding, wound suturing, and ventilator management within the home setting.
- Coordinate discharge planning and case management activities to ensure seamless transitions between care settings.
- Provide education on disease management including dementia care, Alzheimer's care, pain management, and medication adherence for patients and their families.
- Collaborate with interdisciplinary teams to develop personalized care plans that address acute pain, infectious diseases, or chronic conditions like geriatrics or developmental disabilities.
*Skills*
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and CPT coding for accurate documentation and billing.
- Skilled in patient assessment techniques including vital signs measurement and diagnostic evaluation.
- Expertise in specialized areas such as hospice care, palliative care, pediatric or neonatal ventilator management, and post-acute or sub-acute care environments.
- Excellent communication skills for triage calls and health coaching; ability to work independently while collaborating effectively with healthcare teams.
