Home Health Admissions Nurse (RN) serving as the clinical leader for Start of Care (SOC) admissions throughout the Twin Cities Metro. Responsibilities include performing detailed nursing assessments, completing accurate OASIS documentation, developing individualized plans of care, educating patients and families, coordinating with physicians and interdisciplinary care team, and ensuring smooth transitions from hospitals and facilities into home health.

Requirements

Active Minnesota RN license
Active Minnesota Registered Nurse (RN) license
Strong clinical assessment and critical thinking skills
Home health and OASIS experience strongly preferred
Exceptional communication and patient education abilities
Ability to work independently and manage a field-based schedule
Reliable transportation
