Perks and Benefits*: * Licensed Practical Nurse position. * * Earn up to 1% wage increase every quarter * 401K Retirement Plan with Safe Harbor matching contribution * Length of Service Bonus Program of up to $5,000 * Employee Referral Bonus of up to $1,000 * Access to earned wages prior to payday * Generous PTO Plan * Career Development * An employee engaged scheduling system * Affordable Medical, Dental, Vision, Supplemental Benefits * Sinceri Senior Living Discount Marketplace * Walking Spree rewards for healthy habits Oh, and did we mention an amazing Team Environment? *Some benefits may vary depending on position and employment status Essential Functions: * Monitors the daily care of all residents * Performs skilled nursing tasks as defined by company policy and procedure, and as allowed by state law * Documents observations, concerns, and changes in resident conditions, incidents, and/or accidents, and ensure that follow-up documentation occurs until the issue is resolved. * Communicates resident needs and concerns to family members and physicians * Communicate with family members, physicians, and other outside healthcare providers * Records and implements physician orders * Ensures that information and interventions recorded on resident service plans are implemented by Caregivers * Ensures that Medication Aides are maintaining an orderly medication cart, medication room, and accurate medication administration records * Receives reports from caregivers before the end of the shift * Provides status reports to oncoming Staff Supervisory/Management: * Assist in shift assignments and ensures that assigned duties are completed in a satisfactory manner * Monitors care staff performance for compliance with established policies and procedures, and to ensure quality resident care * Manages shift coverage when staff members call in sick for the oncoming shift * Responsible for scheduling and other staffing duties, as requested * Deals appropriately with a family member and visitor concerns or complaints, documenting the issues and actions taken through the appropriate reporting mechanism * Manages minor physical plant issues that arise during assigned shifts, reporting as necessary to Maintenance Director and Administrator Minimum Eligibility Requirements: * Current state LPN/LVN or RN License if required by State * Previous experience working with the elderly in a healthcare setting * Previous supervisory experience preferred * Ability to establish effective relationships with residents, family members, and staff * Strong documentation skills * Ability to represent the facility in a positive and professional manner
