Responsibilities
- Coordinate and manage the care of patients assigned to caseloads in an outpatient urgent care environment.
- Assess patient needs, develop and monitor individualized care plans, and facilitate appropriate referrals and services.
- Collaborate with the interdisciplinary team to optimize patient outcomes and ensure timely follow-up and transitions of care.
- Educate patients and families on diagnoses, treatment plans, medication management, and self-care strategies.
- Document care coordination activities in the electronic medical record, ensuring accuracy, confidentiality, and regulatory compliance.
- Advocate for patients’ goals of care, supporting equitable access to resources and services.
Requirements
- Current Colorado RN license in good standing
- Graduation from an accredited nursing program (ADN or BSN)
- 1+ year of RN experience with case management, care coordination, or related outpatient experience preferred
- Ability to work effectively in a fast-paced outpatient urgent care setting
- Strong assessment, communication, and collaboration skills
- Demonstrated ability to manage a caseload and coordinate across disciplines
