Responsibilities

  • Coordinate and manage the care of patients assigned to caseloads in an outpatient urgent care environment.
  • Assess patient needs, develop and monitor individualized care plans, and facilitate appropriate referrals and services.
  • Collaborate with the interdisciplinary team to optimize patient outcomes and ensure timely follow-up and transitions of care.
  • Educate patients and families on diagnoses, treatment plans, medication management, and self-care strategies.
  • Document care coordination activities in the electronic medical record, ensuring accuracy, confidentiality, and regulatory compliance.
  • Advocate for patients’ goals of care, supporting equitable access to resources and services.

Requirements

  • Current Colorado RN license in good standing
  • Graduation from an accredited nursing program (ADN or BSN)
  • 1+ year of RN experience with case management, care coordination, or related outpatient experience preferred
  • Ability to work effectively in a fast-paced outpatient urgent care setting
  • Strong assessment, communication, and collaboration skills
  • Demonstrated ability to manage a caseload and coordinate across disciplines
