Hospice RN Case Manager

Full Time RN Case Manager position at Preferred Hospice of Missouri serving patients in the Farmington, MO area. Hospice RN Case Managers see patients in private homes, assisted living facilities, skilled nursing facilities, and anywhere a patient calls home. Performs head-to-toe assessments, provides symptom management and skilled care to patients facing terminal illnesses, provides emotional support and education on death and dying. Supervises LPNs and CNAs, monitors medication lists, assesses for DME needs, provides wound care and other treatments as needed. Monday through Friday, 8am to 5pm.
