What You’ll Do (Role Highlights)

  • Coordinate and manage a caseload of 15-20 patients per week.
  • Provide compassionate care and symptom management.
  • Collaborate with interdisciplinary team members.
  • Complete timely documentation in electronic medical records and ensure compliance with hospice standards.
  • Provide education to patients and families on disease progression and end-of-life planning.

Requirements

  • Registered Nurse license in good standing.
  • CPR Certification.
  • 1 year of Hospice experience is preferred .
  • Ability to manage 15-20 patients per week.
  • Valid driver’s license, reliable transportation and auto insurance..
  • Must be willing to travel within service area to visit patients in their homes and/or facilities.
