What You’ll Do (Role Highlights)
- Coordinate and manage a caseload of 15-20 patients per week.
- Provide compassionate care and symptom management.
- Collaborate with interdisciplinary team members.
- Complete timely documentation in electronic medical records and ensure compliance with hospice standards.
- Provide education to patients and families on disease progression and end-of-life planning.
Requirements
- Registered Nurse license in good standing.
- CPR Certification.
- 1 year of Hospice experience is preferred .
- Ability to manage 15-20 patients per week.
- Valid driver’s license, reliable transportation and auto insurance..
- Must be willing to travel within service area to visit patients in their homes and/or facilities.
