Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy.
- Monitor and document patient status, including vitals, symptoms, and response to therapies; escalate changes to licensed staff.
- Provide basic nursing care and assistance with activities of daily living (ADLs) for residents.
- Maintain accurate documentation in the charting system; ensure timely, complete, and compliant records.
- Collaborate with RNs, physicians, and the interdisciplinary team to support care plans and resident safety.
- Ensure infection control, safety, and comfort for residents during night shifts; participate in rounding and handoffs as directed.
Requirements
- Current Licensed Practical Nurse (LPN) license (active, in good standing).
- Graduation from an accredited practical nurse program.
- 1 year of nursing experience in long-term care or geriatric settings preferred.
- Ability to work 12-hour night shifts and adapt to a rotating schedule.
- Basic life support (BLS) certification required or readily obtainable per facility policy.
- Strong communication and documentation skills; ability to work well within a interdisciplinary team.
