🔎 Position Overview
As a Registered Nurse (RN), you will provide skilled nursing care to patients in their homes, working under physician orders and care plans, while collaborating with a dedicated clinical team.

✅ Key Responsibilities
Deliver skilled nursing care based on individualized care plans
Conduct initial patient assessments and ongoing evaluations

Coordinate care and communicate changes with physicians and team members
Educate patients and families on care and health management
Maintain accurate, timely clinical documentation (including OASIS)
Participate in care planning, case conferences, and quality improvement efforts
Supervise LPNs, CNAs, and HHAs per regulatory guidelines
Support patients’ rehabilitative and preventive care needs
🎓 Qualifications
Active RN license
Graduate of an accredited nursing program
At least 1 year of nursing experience (preferred)
Strong clinical judgment, communication, and organizational skills
Current CPR certification
Ability to work independently and manage time effectively
