The RN Case Manager is responsible for managing the full continuum of patient care, including conducting admissions, completing comprehensive assessments, initiating and managing the plan of care, and coordinating ongoing services for assigned patients. This role serves as the primary clinical decision-maker and point of accountability for each patient, ensuring timely admissions, high-quality care delivery, regulatory compliance, and effective communication with patients, families, physicians, and the interdisciplinary team.
