Are you an experienced Case Management RN with a passion for coordinating patient care and successful discharge planning? This travel opportunity offers the chance to work alongside an interdisciplinary team in a fast-paced acute care setting while making a meaningful impact on patient outcomes. Position: Registered Nurse (RN) – Case Management. Location: Bronx, NY. Assignment Length: 13 Weeks. Shift: 8-Hour Days, Monday–Friday. Schedule: 8:00 AM – 4:30 PM. Guaranteed Hours: 37.5 hours per week.

Requirements

  • Active NY RN License
  • Active NY RN License
  • BSN required
  • Minimum 1 year of inpatient Case Management experience
  • Experience with acute care discharge planning
  • BLS Certification
  • PRI (Patient Review Instrument) Certification
  • SCREEN Training Certification
  • Ability to work independently while collaborating with an interdisciplinary team
