Show full description
Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. The RN PACT Care Manager works in partnership with an assigned panel of patients and team members for effective disease and chronic care management. Uses the nursing process and evidence-based practice to collaborate with the expanded team in developing patient driven care plans for prevention and management of diseases.

Requirements

  • Current full active and unrestricted RN license in a State Territory or Commonwealth of the United States or the District of Columbia
  • English language proficiency
  • Graduate of a school of professional nursing accredited by ACEN or CCNE
  • BSN preferred
  • One year of PACT experience preferred
  • U.S. Citizenship
  • Background/security investigation
  • Drug screening (urinalysis)
