Having served more than 40,000 patients and family members, we are experts in specialized care designed to provide support to you and your loved ones during an advanced illness. Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patient's physician to provide pain and symptom management, as well as social and spiritual support.
The RN Case Manager is responsible for managing the full continuum of patient care, including conducting admissions, completing comprehensive assessments, initiating and managing the plan of care, and coordinating ongoing services for assigned patients.
This role serves as the primary clinical decision-maker and point of accountability for each patient, ensuring timely admissions, high-quality care delivery, regulatory compliance, and effective communication with patients, families, physicians, and the interdisciplinary team.
Requirements
Bachelor of Science preferred; Registered Nurse License required; Minimum 3 years nursing experience, hospice experience preferred; BLS Certification preferred; Nursing experience 1 year preferred; RN required
