We are seeking a detail-oriented and compassionate MDS Coordinator (RN) to join our long-term care team. The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process, completing accurate and timely MDS assessments, developing individualized care plans, and ensuring compliance with federal and state regulations. The ideal candidate is organized, knowledgeable in Medicare and Medicaid reimbursement, and committed to delivering quality resident care.

Requirements

Current, unrestricted RN license
Current, unrestricted RN. MDS experience in a skilled nursing or long-term care setting preferred. Knowledge of CMS regulations, RAI process, PDPM, Medicare, and Medicaid reimbursement. Strong assessment, documentation, and organizational skills. Excellent communication and teamwork abilities. Proficient with electronic medical records (EMR) systems. Long term care: 1 year (Required). Knowledge of CMS: 1 year (Required). Attention to detail: 1 year (Required).
