The Community Care Case Manager RN is accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assume responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Key duties include developing and coordinating seamless transitioning of patients throughout the inpatient and outpatient care process with a focus on discharge planning needs, applying evidence-based practice, discharge planning assessment and implementation of home health services, and serving as a representative on medical center and service committees.

Requirements

  • Current, full, active, and unrestricted RN license in a State, Territory, Commonwealth, or District of Columbia
  • Graduate of a school of professional nursing accredited by ACEN or CCNE
  • English Language Proficiency (basic written and spoken English)
  • U.S. Citizenship
  • Background/security investigation
  • Drug screening (urinalysis)
  • Selective Service Registration for males born after 12/31/1959
  • Seasonal influenza vaccination
