Southern Health Partners is launching a new correctional healthcare program in Cook County, Georgia, and we’re looking for a dependable Licensed Practical Nurse (LPN) to step into the Medical Team Administrator (MTA) role.

As the LPN MTA, you’ll be an important part of establishing a consistent, organized, and patient-focused healthcare program from the start. You’ll provide hands-on nursing care while helping coordinate the day-to-day healthcare needs of the facility.

This is an opportunity for an experienced LPN who enjoys autonomy, organization, teamwork, and making a difference.

What You’ll Do

  • Provide direct nursing care to patients in a correctional healthcare setting
  • Complete nursing assessments and monitor patient conditions
  • Administer medications and treatments as ordered
  • Respond to urgent and emergent healthcare needs
  • Coordinate daily healthcare activities and help maintain continuity of care
  • Communicate with medical providers, correctional staff, and SHP leadership
  • Maintain accurate and timely clinical documentation
  • Support compliance with company policies, clinical standards, and regulatory requirements
  • Help establish organized and consistent healthcare processes at the new site
  • Promote a professional, respectful, and patient-focused healthcare environment
  • What You’ll Bring
  • Current, unrestricted Georgia LPN license
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication and organizational abilities
  • Ability to work independently and confidently make sound clinical decisions
  • Dependability and professionalism
  • Ability to collaborate effectively with a multidisciplinary team
  • Previous leadership or healthcare coordination experience is a plus
  • Correctional healthcare experience is welcome, but not required
