About this role

Show full description
Hospice Nurse Case Manager (RN/LPN) for Home Hospice. Responsible for managing and coordinating comprehensive hospice care for assigned patients including conducting assessments, developing individualized care plans, providing direct patient care in the home, collaborating with the interdisciplinary team, and serving as a clinical resource to patients, families, and caregivers. Full-time, Monday through Friday, 8:00 AM - 5:00 PM with occasional on-call responsibilities.

Requirements

  • Graduate of an accredited Registered Nursing program or Licensed Practical Nursing program
  • Current Georgia Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
  • Valid Georgia driver's license
  • Reliable transportation and ability to travel throughout the service area
  • Minimum of two years of nursing experience
  • Experience providing compassionate care to patients and families experiencing serious illness or end-of-life care preferred
  • Strong assessment, communication, and critical-thinking skills
  • Ability to stand, walk, bend, lift, and assist patients as needed
