New Valley ADHC is a Community-Based Adult Services (CBAS) licensed program that provides services to eligible older adults and/or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization. The RN shall coordinate, administer, and supervise the provision of all nursing and personal care services to program participants at ADHC. The RN shall conduct all nursing assessments, monitor health problems of center participants, and create nursing care plans. The RN shall provide health education and counseling to program participants and their families, conduct health related training to ADHC staff and volunteers, and serve as a nursing consultant. The RN shall refer program participants to appropriate healthcare providers in the community and serve as a liaison with such organizations. The RN shall serve as a key member of the Multidisciplinary Team that provides: Professional Nursing, Social Services, Therapeutic Activities, Physical Therapy, Occupational Therapy, Speech Therapy, Mental Health, and Dietary services to program participants.
Requirements
Valid CA Nursing License
Associate Degree in Nursing
New Graduates Welcome
Bilingual English and Chinese/Mandarin Language (preferred)
Experience preferred
