About this role
Westchester Jewish Community Services (WJCS) is seeking a full-time Registered Nurse to join their outpatient CCBHC team in Hartsdale. The RN plays a critical role in ensuring the coordination of services related to medical conditions, screening/monitoring patients for gaps in preventative services, and physical health conditions related to behavioral health conditions and their co-morbidities. The RN provides screening and monitoring of key health indicators and health risks, as well as clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome and movement abnormalities. The RN will consult with other team members and coordinate care between the psychiatric staff and clinical team, utilizing evidence-based practices and a trauma-informed, person-centered approach.
Requirements
Bachelor's degree in Nursing
Active RN license in New York State in good standing
Computer knowledge and ability to document in an electronic health record
Cleared by NYS Justice Center through fingerprinting for Criminal History Records search
Cleared by Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL)
Ability to work collaboratively with a multidisciplinary team
Experience in behavioral health
Experience with evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery
