- Design and implement patient-specific care plans focused on achieving optimal recovery and enhancing overall health.
- Provide high-quality nursing care in alignment with the patient’s care plan, including comprehensive health and psychosocial assessments to support recovery.
- Monitor patients’ conditions and coordinate health promotion, prevention, and direct care.
- Maintain detailed and accurate clinical records, including all required OASIS documentation.
- Communicate effectively with clinical managers and colleagues to ensure high-quality care.
- Provide training and support to patients and their families/caregivers.
- Function as a key point of contact between patients and community health resources.
- Work alongside physicians and discharge planners to provide health education to patients and caregivers.
- Confirm daily visit schedules to ensure seamless coordination with other staff.
- Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
- Understand and adhere to the agency’s process for managing and addressing customer complaints.
- Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
- Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
Qualifications
- Must have a current RN license (state-specific).
- Valid BLS certification.
- Minimum one year of experience in home health or a healthcare facility.
- Familiarity with Medicare and Medicaid guidelines.
- Possess a solid understanding of professional nursing principles and home health documentation.
- Excellent communication, organizational, and time management skills.
- Must have a valid driver’s license and access to a reliable and insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
