About this role
Administer nursing care for clients in their residence, coordinate care with the interdisciplinary team, clients and their families, and the referring agency. Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health clients. Develop, initiate, and revise the Plan of Care. Provide effective communication to clients, families, team members, and other healthcare professionals. Supervise, teach, and provide clinical direction to other nursing personnel.
Requirements
- RN License
- CPR Certification
- BLS Certification
- 1 year experience as an RN in a clinical care setting
- Home health experience preferred
- Bachelor of Science in Nursing preferred
- Driver's License
- Reliable transportation and auto liability insurance
