RN Patient Navigator - Dialysis Temple, TX | Full-Time | Monday-Friday, 8:00 AM - 5:00 PM Join Baylor Scott & White Health's growing Outpatient Dialysis team supporting approximately 500 patients across three dialysis units. The RN Patient Navigator serves as a patient educator and advocate, helping patients successfully navigate their dialysis journey while supporting quality and patient experience initiatives. Key Responsibilities Conduct new patient orientation and dialysis modality education. Provide targeted monthly patient education. Partner with leadership on quality improvement and patient experience projects. Build strong relationships with patients to improve outcomes. What We're Looking For Registered Nurse (RN) with dialysis experience preferred. Strong patient education and communication skills. Experience with spreadsheets, dashboards, and data tracking.
