Continental Care is seeking a dedicated, detail-oriented MDS Nurse to join our nursing leadership team. This position plays a vital role in ensuring accurate resident assessments, regulatory compliance, quality outcomes, and comprehensive care planning. Position Summary
The MDS/CRC Coordinator is responsible for coordinating and managing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations. This role oversees the accurate completion of Minimum Data Set (MDS) assessments, develops and evaluates resident care plans, supports reimbursement processes, and serves as the facility's clinical resource for Medicare, Medicaid, and quality reporting requirements.
Requirements
- Current, active, unencumbered Nurse (RN/LPN) license in the state of Montana
- Minimum of three (3) years of nursing experience desired
- At least one (1) year of skilled nursing experience desired
- Strong clinical assessment and documentation skills
- Excellent communication, organizational, and time management abilities
- Intermediate computer proficiency, including electronic medical records and Microsoft Office
- Previous MDS Coordinator experience (preferred)
- Resident Assessment Coordinator (RAC) Certification (preferred)
