Shift Monday – Friday, Day Shift; primarily 8:00AM – 5:00PM (40 hrs/wk); other hours/schedule as needed. Job Summary The Nurse Coordinator will work with the Bone Mineral & Metabolism Clinic healthcare team, staff, and other healthcare providers to provide excellent transitions of care within the healthcare setting. Responsible for both clinical and administrative tasks, such as coordinating patient care, developing procedures and providing training/education. The nurse coordinator collaborates with the multidisciplinary team and utilizes clinical outcomes data to support innovative and cost-effective changes to clinical practice. The nurse coordinator is accountable to patients and families for the processes and outcomes of care during an episode of care. The nurse coordinator is accountable to peers for the advancement of group practice in the practice setting and to the nurse/clinic manager for the totality of work performance. This nurse will be competent in the skills/knowledge required and fully participate in practice setting governance.
