Looking for a valuable team player to join our hospice program. We are seeking a compassionate and dedicated *Hospice Nurse Clinical Manager* (Patient Care Manager) to join our mission‑driven hospice team. This leadership role is ideal for an RN who brings both clinical expertise and a heart for supporting patients and families through one of life’s most meaningful transitions.

*About the Role *As our Hospice Nurse Clinical Manager, you will guide and support our clinical staff, ensure high‑quality patient care, and help uphold the values that define our organization. You’ll work closely with an interdisciplinary team to create an environment where patients and families feel supported, respected, and cared for with dignity.

  • Active RN license (state‑specific)
  • RN BSN preferred
  • Minimum 5 years of nursing experience, or 3 years with applicable hospice plus home health, or clinical leadership experience
  • Strong leadership and communication skills
  • Deep understanding of hospice philosophy and best practices
  • Ability to support staff and families with empathy and professionalism
