About this role
A well-established skilled nursing and rehabilitation center in the Wakefield MA area is seeking an experienced and detail-oriented MDS Coordinator to join our team full-time. The ideal candidate will be a licensed nurse with strong clinical assessment and documentation skills who can ensure accurate and timely completion of MDS assessments and compliance with all state and federal regulations.

Requirements
- RN License (Massachusetts)
- LPN License (Massachusetts)
- Current RN or LPN license in Massachusetts (required)
- Minimum 1 year of MDS experience in a skilled nursing facility (preferred)
- Strong knowledge of RAI process, PDPM, and Medicare/Medicaid guidelines
- Excellent attention to detail, communication, and organizational skills
- Ability to work independently and as part of an interdisciplinary team
- Skilled nursing facility experience
